Instructions for Completing the Initial Patient Package
If you are completing the Initial Patient Package, please be sure to initial each highlighted initial line appearing in both consent forms included in the Initial Intake Form package.
Please complete this form electronically, print it, and bring it with you to your appointment. If you do not have a printer, you can fill this form out at the office. Please arrive at least 15 minutes before your appointment to do so.
Instructions for Completing the Patient Recertification Form
If you are completing the Patient Recertification Form, please be sure to initial the text opt-in box so that we may send you prescription expiration reminders. Otherwise, you run the risk that your prescriptions will lapse.
If you are seeing us virtually, please complete the credit/debit card information section. If you are seeing us in person, there is no need to do so.
If you have an in-person visit, please complete this form electronically, print it, and bring it with you to your appointment. If you do not have a printer, you can fill this form out at the office. Please arrive at least 10 minutes before your appointment to do so.
